This months training will be phenomenal.  Each member of this diverse panel has their own distinct view of the LGBTQ Community.  The discussion that followed the last training panel was honest, deep and moving.  I expect a similar experience with this training.

This will be our last training at Cedar Point Recovery Center.  I want to thank them for their gracious hospitality and the support the staff offered at every training.          

I am extremely delighted that Darlene Davis is facilitating a two day Law and Ethics Training in July.  She developed the curriculum to address the needs of interns just entering the field and seasoned therapist. The training is about 75% full.  Please see the newsletter for more complete information.
       
The response to the Mission Statement ballot has been great.  The ballot will remain open for another 24 days.  Thank you to everyone for your participation. 

Alexis Clemons and Ann Leber, members of our Sacramento Valley Chapter Board, are planning a fantastic event in November to celebrate our forty fifth anniversary.  More details will be shared in our up coming newsletters.

The chapter thought it would be helpful to keep you updated on new laws, legislative pursuits or actions, as well as ongoing legaland ethical dilemmas we all face in our career as Licensed Marriage and Family Therapists, Interns, and Trainees. Please feel free to ask questions that you have and I will do my best to investigate and post your answer in the newsletter. Please note that articles are based on information from CAMFT and/or the BBS and have been researched to the best of my ability. I watched the BBS meeting held on May 16, 2016. There were three bills that are being considered that are worth mentioning. I have listed the bills and added my own thoughts and opinions and not necessarily those of the BBS.

AB1808 WOOD Minors:Mental Health Services

The current law only allows Interns to see Minors 12 yrs or older without parental consent under certain circumstances. See HSC 124260(b) This bill would include trainees and clinical counselor trainees. There is some controversy over whether this is a good idea. There is concern that a trainee needs to consult with their supervisor as soon as reasonably possible or within 24hrs. The wording consult is troublesome. Many times there are benign reasons a trainee will see a minor without consent and there is not a need to consult with a supervisor until the next supervision meeting. Trainees are typically in supervision 2-3times a week based on the BBS supervision ratios required by law.  I wonder whether notify a supervisor as soon as reasonably possible or within 24hrs is a better description of what should happen. This one word allows the supervisor to be aware of what is happening and can determine whether a consultation is needed immediately or if the subject can wait until the next supervision meeting. If it is a case where there is danger to self or others then I would hope a trainee is consulting with their supervisor. There will be a provision in the bill that would speak to harm to self or others and the word consult is appropriate. Wondering what you think? Feel free to contact me or write to your BBS representative for comments. This may move through legislature soon!

AB1863 WOOD Medi-Cal:Federally Qualified Health Centers: Rural Health Centers

This bill would allow Medi-Cal reimbursement for covered mental health services provided by a marriage and family therapist employed by a federally qualified health center or a rural health clinic. Existing law only specifies reimbursement for physicians, physician assistants, nurse practitioner, certified nurse-midwife,licensed clinical social worker, visiting nurse, or dental hygienist. This serves as a disadvantage to FQH’s and RHC’s to hire marriage and family therapists. These clinics are in desperate need for services. There were several agencies that came to the BBS meeting to state that they desperately need the clinical services by LMFT’s and are in support of the bill.Unfortunately this is one where NASW, CA is opposing this bill. Agencies need to speak up on their experiences on this matter!

AB2199 CAMPOS Sexual Offenses Against Minors: Persons in a Position of Authority

Under current law specifies that a person 21 or older who engages in unlawful sexual intercourse with a minor under age 16 is guilty of either a misdemeanor or a felony and that is punishable by imprisonment for a term ranging from one to four years.Penal Code §261.5(d)There are other violations listed under this penal code for activities other than sexual intercourse. The point of the bill would be to add a sentence enchantment for adults charged with felony statutory rape when they are in a position of authority. These people in positions of authority know their victims ages as well as their vulnerabilities.

AB2507 GORDON Telehealth: Access

This bill would require that a health care service plan or health insurer cover patient services provided via telehealth to the same extent as services provided in-person. It would also specify the communications platforms that are acceptable for telehealth.Currently in California it is up to the health service plan to decide if telehealth is a covered service. Many other states allow the same coverage for telehealth as it does for in-person mental health services. Tele-health is becoming more common in the mental health field and is allowing those in rural areas or those that cannot come to our offices to receive much needed mental health services.

Remember these BBS meetings are open to the public and they need our input before laws go into place that may impede the community from receiving the services they need.

  May Presentation Summary

Transcranial Magnetic Stimulation an Innovative Treatment for Depression (2 CEs)

Presenter: Dr. Richard A. Bermudes, MD


          Depression is destructive in the life of an individual, and the inability to manage the effects of major depressive disorder with medication is discouraging.Transcranial Magnetic Stimulation (TMS) may be a preferable treatment option for those who have had little to no success with medication. In this month’s SVC-CAMFT lecture, Dr. Richard A. Bermudes identified difficulties clients with treatment resistant depression face and provided a succinct presentation of TMS’s role in the treatment process.

     TMS is a technology that developed in the 1990’s as a treatment for depression. It uses strong magnetic pulses to stimulate the brain and the coil is positioned to stimulate the dorsolateral prefrontal cortex an area that is under active in clinical depression.  TMS is FDA cleared in adults with major depression who have not responded to one or more antidepressant medications. Studies indicate that 40% of patients treated with medication do not obtain remission even after being prescribed several trials of antidepressant medications. For example, if a patient has not responded to two antidepressants their chances of reaching remission with the third antidepressant is 16%.  Research outcomes showed that improvement rates were high after undergoing TMS treatment, with a significant number of individuals reporting a decrease in symptoms. Thirty percent of participants went into remission after receiving the full course of treatment in a subsequent study. Recorded answers also indicated a shift from severely depressed to mildly depressed or less in eighty percent of participants presenting a strong case for the use of TMS for treatment resistant depression.

           Course of treatment, as described by Dr. Bermudes, was non-invasive, conducted in an outpatient setting, and occurred over a six-week period of time. Within the program, clients participated in 20-50 minute procedures five days a week. At each visit, clients were seated in a chair similar to those found in dentists’ offices and a magnetic coil was placed over the forehead. During treatment individuals had the opportunity to work on activities supporting the therapeutic process.

 

Keypoints to consider when suggesting this treatment to clients are:

 

  • ·     TMS is indicated for adult patients with major depression who have not responded to one or more antidepressants
  • ·     TMS is 2-4 times more effective than medication treatments in patients who have not responded to two or more antidepressants
  • ·     Most insurance companies cover TMS after four or more trials of antidepressant treatment although coverage policies vary
  • ·     TMS is commonly added to other partially effective treatments such as medication and psychotherapy
  • ·     TMS is very effective to help patients maintain remission in those patients who have responded.

Workshop Information:

Four panel speakers, all members of the LGBTQ community in northern California, will speak individually to the dynamics of race and health disparities in the LGB community, non-binary and non-conforming gender identities, what therapists need to know about trans rights, and the status of mental health access to LGBTQ community members. The second hour of the presentation will consist of a question and answer format.

Learning Objectives:

  • Understand the dynamics of race, racism issues, and health disparities in the LGB community
  • Increase awareness of non-binary and non-conforming gender identities and associated issues
  • What therapists need to understand about trans rights
  • The status of mental health access for members of the LGBTQ community

Presenter Bios:
Reggie Caldwell, MSW, LCSW
is a clinical social worker with more than 20 years experience in local and state public health. He is a policy and program coordinator for the California Department of Public Health where he develops and implements health policy targeting racial and ethnic health disparities. He is also a psychotherapist for individuals and couples.

Gem Embrey (he, him, his/they, them, theirs) is a trans masculine gender non-conforming FTM predominantly fem attracted pansexual. He currently works in the Pride Center at Sac State as the Academic Engagement Assistant where he is a graduating senior in psychology. His work and study are predominantly focused on creating more inclusive environments, LGBTQIAA* issues and experiences, and gender equality.

Rachael Hudson was on the planning board for the Gender Health Center in Sacramento and was named Operations Manager, the only full-time employee, when GHC opened its doors in July 2010 and is now the Outreach Director. Rachael is a trans  woman living in Sacramento and became a board member of the Sacramento Gender Association in 2004. In 2007, she co-founded TransFamiliesSacramento and SOFFA-T support groups. She now facilitates both TransFamilies,which meets with PFLAG, and the TransFem support meeting.

Arden Tucker is a Marriage and Family Therapist Intern currently working at Life Practice Counseling Group. She has been in counseling for over 20 years with various agencies and has been a consumer mental health advocate in the Sacramento mental health community for 12 years. Among the many committees and collaboratives she serves on, inclusion of culturally sensitive service delivery to the LGBTQ community is at the forefront of concerns that Arden voices in all her involvement.

Hiding in Plain Sight

I am here hiding in plain sight.
No one knows me but they all think they do.
I hear everyday what people would think of me if they knew who I was.
I try to fit in, in a world that does not accept me.
Such stereotypes and discussions of what people like me are, make me angry
and ill, yet if I speak up what will they think? Will they think I am like
the people who disgust them, that they feel are twisted or scary?

Comments are constantly made, I have a thick shell, I am used to it.
Even then some hurt.
I want to speak up, but how?
If I say I know people like me and these people are not the stereotypes
they talk about, will they think the same things they are saying about them about me?
What if they figure out that I am one of these people?

From the past, I know I cannot say anything for everything will change and be even worse.
Friendships and work relationships will be strained, everyone will be afraid to joke or
say anything and my stomach will knot up every day I have to be there and feeling the stress.

I would have done anything to change who I was to not live this way every day of my life.
So many things that everyone else takes for granted are not the same but they think it is
and that people like me want special rights.

I just want to be happy like everyone else.
I want to not live in danger every single minute of my life.
I do not want to fear for my children's lives.
I do not want to have to edit so much of my life when someone asks me a question and worry I messed up.
I cannot invite people to dinner at my house or a BBQ because I might be found out.
I have to be very limited in my friendships outside of controlled situations.
I can never drink or I might slip up.

Think of everything you hold dear in your life,
your children, your family, your spouse, your friends
and imagine that nearly everyday you are challenged on why
you think you should be allowed to have those things.
I am no different than you.
I want to have those things too.

Who am I?
Maybe I am....
your co-worker.
your friend.
a person who overhears your conversation,
your boss,
your family member,
your worker,
a loved one,
or anyone else..
Because I am hiding in plain sight you may never know
that I am gay.

Review:
This picture book by S. Bear Bergman is told in the voices of different kids questioning society's gender roles.  I appreciated the different types of kids that reflect their different ages (baby to late elementary) and interests. The illustrations work well with the text. Included are children of color and even a disabled child.  Great to give comfort to a child struggling with what they want to do, become or play with.
